Making Geriatric Care Managers a Part of Your Care Coordination Plan

Making Geriatric Care Managers a Part of Your Care Coordination Plan

September 24, 2012 - 10:00 am

An elderly person’s family is his or her best pillar of support, especially when he or she becomes fragile and needs assistance in the home. However, while they mean well, family members can sometimes lose sight of what is best for the senior when everyone wants to be involved with care. This is when tempers can begin to rise and family goes from being supportive to stressful.

In times of stressful family care coordination, a geriatric care manager (GCM) can help. GCMs are trained to assist families in determining what kind of care an elderly person requires and how to best coordinate long-term care. They can also act as mediators when sons, daughters, grandchildren, aunts, uncles, etc. find themselves at odds when trying to plan the eldercare of their loved one.

GCMs & Home Care

One of the benefits of using a GCM to help coordinate care is that they are professionals trained to work with many different types of people and organizations. A GCM can help coordinate medical care, work with attorneys and coordinate with your chosen in-home care agency.

At Partners in Healthcare in Orlando, we work side-by-side with GCMs to help support families in making the best care decisions for their loved ones. Contact us today for more information at 407-788-9393, and follow us on Facebook at HomeCareOrlando.
