Required Documentation for Employment
The following items are required at or prior to Orientation:
- Level II FDLE/FBI background check (we will verify)
- RN, LPN, CNA License or HHA Certificate
- Current CPR certificate (hands on training)
- TB screening
- Social Security Card
- Drivers License
- Auto Registration and Insurance
- Direct Deposit account
- Copies of current CEUs
- Business references
- We will conduct on-site drug screening
- Driving, federal, state, licensing record checks
